En internationell studie på över 160 spädbarn med deras första febrila urinvägsinfektion har gett omfattande molekylära data om akut pyelonefrit, en allvarlig njurinfektion. Studien, som inkluderade barn från Singapore och Sverige, visade att infektionen inte bara orsakas av bakterier utan också av en dysreglerad immunreaktion. Den identifierade nya precisionstargets och biomarkörer, inklusive genaktiviteter kopplade till vita blodkroppar, och en fyragenspanel som kunde förutsäga njurengagemang med 94% noggrannhet. Resultaten utmanar tidigare uppfattningar om njurärrbildning, som istället kopplas till sena immunaktiviteter snarare än akut infektion. Studien ger en grund för utveckling av icke-antibiotiska behandlingar för bakterieinfektioner och stärker Hamlet BioPharma’s position inom immunterapier. Forskningsresultaten stöds av flera internationella finansiärer och publiceras i Life Science Alliance.

Hamlet BioPharma, in collaboration with Lund University and Linnane Pharma, has identified four candidate molecules aimed at treating infectious diseases by targeting the body's immune response rather than the bacteria directly. These molecules are designed to reduce harmful inflammation and enhance the body's natural defenses, allowing for the elimination of bacteria without relying on antibiotics. The four molecules include NlpD, IRF7 siRNA, Lon, and IL-1RA, each with a unique mechanism for modulating the immune response. This approach aims to restore immune balance, reduce antibiotic use, and remain effective against antibiotic-resistant bacteria.

The thesis is based on four papers, three published in peer-reviewed journals and one manuscript. The first paper introduces a new concept for cell death, focusing on how Alpha1H targets the endoplasmic reticulum (ER) in tumor cells, potentially explaining the low toxicity of the alpha1H complex. The second paper discusses a clinical study on bladder cancer patients, showing that Alpha1H significantly reduces tumor size and number, with strong dose-dependent anti-tumor effects. The third paper examines the mechanism of rapid tumor cell shedding in patients treated with Alpha1H, suggesting that it targets cell adhesion mechanisms. The fourth paper explores the combination of Alpha1H with chemotherapy in a murine bladder cancer model, showing that Alpha1H reduces tumor progression and enhances chemotherapy effects. These findings support further development of Alpha1H as a therapeutic strategy for bladder cancer.
